Application for financial aid to GMSPD requires the following three items:

• Completed and signed application form found here
• IRS form 1040.  For privacy purposes, you may black out all social security information.
• Recommendation for this scholarship from your school teacher/principal/advisor addressed to the Gifted Music School.

All materials are due  one month before the start of the semester for which you are applying for aid. Please mail all materials to GIFTED MUSIC SCHOOL, PO BOX 9841, SLC, UT 84109
